I get sweaty palms, teary eyes and try to take my thoughts far away when I go to the dentist and that is just for a cleaning. He gave me (the good kind of) gas twice yearly for my cleanings. I would have had my teeth cleaned monthly, but my insurance wasn't biting (little dental humor). And, praise be to God/Allah/Buddah, et al for the mighty pharmaceutical companies! Is that an option? Because you definitely sound like a candidate for it.
